It means that the On Board Diagnostics computer is receiving information from the sensor array that indicates a condition that suggests a problem with the EGR (Exhaust Gas Recirculation) circuit. The EGR valve on that model consists of two parts - a vacuum manifold and a valve body. The hoses between the valve body and vacuum manifold could be leaking or disconnected, the gasket between the EGR valve and intake manifold could be bad or the valve itself could be stuck open or closed. If you have a handheld vacuum pump, you could attach it to the vaccum port and see if the valve will pull in when vacuum is applied. If you're not handy, it will be necessary to either find someone who is or take it to a trusted mechanic. The part costs about $100 from automotive parts suppliers.
