The stiletto heels makes the woman taller so she looks thinner, and tenses her calf muscles so her legs look better, and tightens her but so that also looks better. That's why chicks with stiletto heels on are hotter. (they go up at least one point. e.g. 7.5 to 8.5)

Guys don't care about high heels, they don't think its sexy unless that's all they're wearing. We do think they can be elegant for the occasion. We will point out if we like your heels, that's of course if they stand out, otherwise we are more focused on you, unless your wearing funky shoes, mismatch or out of occasion, we might focus on shoes.

Most men probably do, but it's entirely a matter of personal taste. I personally only find them attractive if they are high-heeled 'open' shoes (i.e. with only straps for uppers) worn on bare feet, and am far more turned on by flat Roman-style sandals worn without coloured nail varnish on the toes....
